睡眠時無呼吸現象の中枢神経機序

研究概要	ハロセン麻酔下で除脳したネコの脳幹に微少電極を刺入し、脳幹内でシステマティツクに微小電極を移動させながら微小電気刺激を加え、後肢筋緊張の減弱効果と呼吸運動の強い抑制効果をもつ部位(橋中心被蓋野背側部,DTF)を同定した。別の微小電極をDTFよりも吻側の橋網様体に刺入し、それを記録電極としてDTFの単発刺激によって逆行性応答をするニュ-ロンを同定した。DTF刺激によって逆行性応答を示すニュ-ロンは吻側橋網様核(PoO)はほぼ限局して認められた。それらのニュ-ロンは、安静時における自発発射がほとんど認められず、通常は活動が極めて弱いものと推定された。次に、逆行性ユニット電位の記録部位にカルパコルを封入したガラス管微小電極を刺入し、圧注入法によりコリンアゴニストであるカルバコルの微量注入を行った。その結果、抑制の時間経過こそDTF刺激とは異なるものの、カルバコルの注入によっては、DTF刺激と質的にはほぼ同様に効果、すなわち筋緊張と呼吸運動の同時抑制効果が認められた。ハ-バ-ド大のHobsonらによる研究では、慢性ネコのPoO近傍へのカルバコル注入によって、ほぼ確実にREM睡眠が誘発できることがわかっている。そして、昨年までの我々の研究によって、PoOからDTF、延髄巨大細胞性網様核を介して脊髄へ到る下行抑制系が同定された。しかもその抑制系は、全身の筋緊張の減弱(atonia)ばかりでなく、横隔膜・外助間筋などの呼吸筋活動も強く抑制し、運動系の全体に及ぶ強力な汎化抑制系であると推定された。DTF刺激に基づく無呼吸(apnea)からの回復過程の解析および以上の実験結果から、ここで同定したPoOに起源を有し、脊髄へ下行する汎化抑制系は、睡眠時とくにREM睡眠に活動を高め、呼吸運動を含む運動系の全体を抑制する。そして、もし中枢化学受容野に機能障害があれば、持続した無呼吸状態を誘起するものと推定できた。
