慢性関節リウマチの遷延化に及ぼすインタ-フェロンα低値の病理病態学的研究

研究概要	高感度のラジオイムノアッセイを用いてヒト血中インタ-フェロンα(IFNα)を定量し、IFNαが血中で分解され従って従来の生物学的アッセイで検出されないこと、血中IFNαは小児で低く青年期で最高値で以後加令と共に漸減し、その血中レベルは個人でほぼ一定しているが、ウィルス肝炎(A型)、ウィルス感染、麻疹等に比べ、特に免疫処理に欠陥の予想される激症肝炎や成人T細胞白血病等で著しい低値で、これにIFNの治療が有効であること。また血中IFNαは慢性関節リウマチ(RA)のほぼ全例で疾患活動性を問わず低値で、末梢血のIFNαの産性能も低く、且つIFNαの治療は二重旨検・一年から三年の臨床試験にてRAの炎症所見を有意に改善せしめた。こうしたIFNαの低値の原因を知るために、RA患者のリンパ球・マクロファ-ジにおけるIFNαの産生状況を、ラジオイムノアッセイによる物質レベル、ノ-ザンブロット法によるメッセ-ジレベルにて検定すると、いずれも低値であると考えられた。さらにインタ-フェロンαにはTNFαとの相乗効果をもって骨芽細胞を制御し、RAにおける骨粗鬆症化の抑制作用が推定された。実際IFNα投与によって、血中カルシウムが有意に上昇する反面、PTH、オステオカルシン等の調節因子に変動のないこと。さらにIFNαはアルカリフォスファタ-ゼ値を低下させることから、今回の研究は発展的にRAの骨粗鬆症の原因・治療の解明へと向っている。即ち、IFNα及びIFNαには骨粗鬆化の抑制効果があると考えられた。この他、本研究費より(1)SLEにおける抗DNA抗体産性様序の解析。即ちサプレッサ-インデュ-サ-細胞異常の存在(業績)。(2)亜鉛による自己免疫の干渉効果という古くからの現象がインタ-ロイキン2に関係している事。(3)滑膜細胞はEpidermal Growth Factor陽性で、間葉系に近い性状をもつ事等を報告した(業績参照)。
