家族性クロイツフェルト・ヤコブ病のプリオン蛋白多型性に関する研究

研究概要

クロイツフェルト・ヤコブ病(以下CJDと略す)の発症因子としてプリオン仮説が提唱され、1989年にカリフォルニア大学と我々のグル-プが家族性CJDであるゲルストマン・ストロイスラ-症候群(GSS)で102番のプロリンがロイシンに置換していることを報告した。昨年度みいだした117番,129番,200番,168bpの挿入変異をもつ患者が、どのように臨床像および病理像を呈するのかを検討した。プリオン蛋白の病理切片での検出感度を上げるため、新たに加水分解法を開発した。本法により、プリオン蛋白遺伝子に変異のないCJDではシナプス構造にプリオン蛋白の蓄積を認めることが明らかとなった。本法を用いて検出可 能となったプリオン蛋白の形態は2種類に分類でき、主にシナプスをおかすシナプス型と主に細胞外にアミロイド斑を形成するプラ-ク型である。シナプス型の沈着が認められるのは、変異のないCJDとコドン200番のCJDであり、プラ-ク型の沈着は、コドン102,117,129,の点変異を有するCJDおよび168bpの挿入変異の症例であった。シナプス型の症例は発症後の臨床経過が短く、約3ヶ月でakinetic mutismになるのにくらべ、プラ-ク型では臨床経過が長く、いわゆる寝たきり状態となるのに5〜6年と長期間を必要とする。これらの結果より、プリオン蛋白の一次構造は、直接アミロイド沈着の部位に影響を及ぼし、アミロイド沈着の部位により臨床経過が異なるという結果が明らかとなった。現在、我々は、全く新しい変異をみつけつつあり今後ともプリオン蛋白遺伝子のOpen reading franeを中心とした遺伝子解析は、CJDやGSSの病因探求にとり不可欠なものと思われる。
