子宮頸部扁平上皮の増殖・分化調節機序の解析に基づく頸癌発生過程の研究

研究概要	子宮頸部における局所感染防御機構の有無とその性周期による調節機序の解析の検討結果は、頸部扁平上皮のランゲルハウス細胞の分布の免疫組織学的検討で健常婦人の頸部扁平上皮には23.8±2.6(M±SE)/highpowerfield(HPF)個のランゲルハンス細胞が分布し、子宮頸部上皮内腫瘍近傍で正常と思われる扁平上皮には9.4±1.7HPF個と有意に少ないことが判明し、頸部上皮内病変を有する局所の免疫機能の低下が示唆された。頸管腺上皮から分泌される頸管粘液の中には免疫グロブリンIgA,IgGが含まれ、腹腔と外界が交通している女性生植器において液性免疫機序が感染防御に重要な役割を果たしている可能性が考えられるが、実際頸管粘液中のIgA,IgG値はともに卵胞期に高く、黄体期・妊娠中・経口避妊薬服用中はこれよりも有意に低値で、また月経周期の中では排卵期に最も高値を示した。この結果は、液性免疫機序が性周期の中で調節されている可能性を示唆し、卵胞期の中でも排卵期という本来性交が行われる時期にその機能が最も高まっているのに対し、黄体期・妊娠中・経口避妊薬服用中にはその機能が低下していることを示唆する。このことは、ヒトにおいても発情期に相当する排卵期には大量の頸管粘液を分泌し、その局所免疫機能が備わっているのに対して、その他の時期はやや無防備であることを示唆し、ヒトの獲得した性周期とは無関係な性交文化は子宮頸部における頸癌発生に重大なかかわりがあるとされているhuman papilloma virusなどの感染にとって不都合なものである可能性が示唆された。また、子宮頸癌患者の内分泌環境の検討では、頸癌初期病変を発生する宿主の中分泌は女性として極めて正常なものであり、大部分の頸癌は黄体期を有する通常の性ステロイド環境の下で発生する可能性が示唆された。
