乳幼児心疾患の心室筋における心房性Na利尿ペプチドの免疫組織学的研究

研究概要	東京女子医科大学病理学教室で解剖を行った、乳幼児心内膜線維弾性症(EFE)14例、拡張型心筋症(DCM)7例、肥大型心筋症(HCM)4例先天性心疾患15例、および対照として同年令層の非心疾患25例について抗ヒトαANP抗体を用いて免疫組織染色を行った。それぞれ心房および心室の標本を10%ホルマリンで固定し、パラフィン包埋ブロックより標本切片を作製し、酵素抗体法(AvidinーBiotin Peroxydase Complex法)により行った。その結果、心房筋では全例にANP免疫活性陽性顆粒が認められた。心室筋ではHCM症例、先天性心疾患例と対照例においてはANP陽性細胞を認めなかったが、EFE の10/14例(71%)、DCM5/7例(71%)にANP陽性細胞の発現をみた。陽性細胞はいずれも左室心筋に認められたが、一部の症例では右室心筋にも同様に陽性細胞が観察された。陽性細胞は心内膜側に多く分布していた。免疫反応陽性顆粒は細胞質内にびまん性に認められる場合や、細胞内の核周囲に集中していたり、細胞の辺縁に認められる場合もあった。心重量とANP陽性細胞の発現には関連はみられなかったが、左室容積はANP陽性心筋を有する心蔵と、ANP陽性心筋を認めない心蔵とで有意の差がみられ、それぞれの左室容積係数は、5.9±1.3および4.3±0.9であった。ANP陽性例において、心筋細胞の横径をANP陽性および陰性細胞について計測し平均横径を比較すると、前者のほうが後者に比べて有意に大きいことが示された。心室筋におけるANPの発現は、高度の心室拡張と関連があることが推測された。
