大腸癌のDNA診断に関する研究

研究概要	遺伝性、非遺伝性の進行大腸癌において高率(〜44%)のras遺伝子突然変異を検出した。染色体5qの対立遺伝子欠失はFAP遺伝子が存在する5q21ー22の領域で高率(〜56%)に認められ、この領域にマップされる多型性プロ-ブYN5.48が最も有用であり、61%の欠失を検出した。17pでは64%の癌の欠失が認められ、YNZ22が最も高い69%の欠失を検出した。18qは52%の欠失率で、OS4は63%の欠失を検出した。22qに関しては欠失の合計が40%で、IGLCが最も高い50%の欠失を検出した。更に、家族性大腸腺症患者に発生した大きさの異なる腺腫や粘膜内癌についてDNA変化の解析を行った。低〜中異型性腺腫では異常は殆ど認められてなかったが、高異型性腺腫ではras突然変異(36%)と5qの欠失(20%)が、粘膜内癌ではras変異(26%)と5q欠失(26%)と17q欠失(38%)が、検出された。以上の結果から、腺腫の進行にFAP遺伝子のヘテロ接合性消失とras変異が関与し、腺腫から早期癌への転換に17pの欠失が、さらに、早期癌から進行癌への進展に18q,22qの欠失が関与することが示唆された。一般集団に発生した中異型性腺腫、高異型腺腫瘍、粘膜内癌、浸潤癌についても解析を行った結果、家族性大腸腫症の場合と同様の結果が得られた。 17pに存在する53遺伝子の変異をPCRーSSCP法によって解析したところ56個の変異が見いだされ、p53遺伝子の変異と欠失が腺腫から早期癌への転換に関与することが示唆された。また、p53に変異を有する癌の85%が抗p53抗体を用いた免疫組織染色に陽性となり、この組織染色が癌細胞に極めて有用であることが分かった。
