脱神経筋と神経血管柄付筋肉移植における筋細胞の変化について

研究概要

1.実験的脱神経筋と神経血管柄付筋肉移植,臨床的神経血管柄付筋肉移植における移植筋細胞の形態変化
実験的脱神経筋では,筋線維の変性の後,筋芽細胞と未熟筋管細胞が出現したが,その後,未熟筋管細胞はその数が減少する傾向をしめした。しかし,1年以上経過しても筋細胞が消失することはなかった。実験的神経血管柄付筋肉移植では,移植筋線維の崩壊の後,筋芽細胞と筋管細胞が出現し,ほぼ完全な筋線維となった。臨床例の神経血管柄付筋移植後長期経過した筋では,正常筋線維の間に筋管細胞,筋芽細胞がみられた。この結果,脱神経筋と血管柄付筋肉移植においては,筋細胞は一旦変性した後,筋芽細胞が出現し,これが融合して筋管細胞となる。血管付群では,その後分化型筋線維へと分化することが解った。しかし,臨床例では,術後長期間を経ても移植筋内には筋芽細胞とか筋管細胞などの未熟な筋細胞が比較的多く存在するものと考えられる。
2.臨床例の陳旧性顔面神経麻痺に対する新しい治療法としての“神経血管柄付大腿直筋移植術"による一期的再建法の確立
実験的脱神経筋の検索結果より,脱神経後長期間を経過しても再生中の未熟筋細胞が残存していることが判明した。そこで臨床的な陳旧性顔面神経麻痺の患者に対し,現在,二期的になされている神経血管柄付薄筋移植術に変わり一期的大腿直筋移植術をこれまでに4例に対して施行した。その結果,術後1年前後で移植筋が随意的に収縮することが判明した。本法はその簡便さから,今後広く普及することが予想される。
