パーキンソン病における髄液中の各種神経伝達物質の変化とその臨床的意義

研究概要	研究目的: パーキンソン病の主病変は黒質にあるが、セロトニン系ニューロンの縫線核の病変を伴うことも知られている。セロトニンの髄液中濃度は低く、その測定は困難であり、その代謝産物である5-HIAAがセロトニン系の動態の指標として扱われてきた。最近我々は、セロトニンの髄液中濃度を測定する方法を確立した。そこで、パーキンソン病患者16例を対象に、高速液体クロマトグラフィーにより髄液中のセロトニンおよびその関連物質の濃度を測定し、臨床症状との相関を検討した。研究結果: セロトニン、5-ヒドロキトリプトファン、キヌレニン、3-ヒドロキシキヌレニン濃度は、パーキンソン病では有意に低下していた。セロトニン濃度の低下は、Hohen and Yahrの重症度と有意の相関であった。個々の症状と関係をみると、セロトニン濃度の低下は、筋固縮、無動、すくみ足の重症度と有意の相関があったが、とくにすくみ足との相関が強かった。振戦の程度とは関係がなかった。考察および今後の研究の展開:以上の結果は、パーキンソン病の諸症状にセロトニン系の異常も関与していることを示している。今後、セロトニン系とドーパミン系の相互の関係、L-dopa投与がセロトニン系に及ぼす影響などについて検討する。
