パーキンソン病における髄液中の各種神経伝達物質の変化とその臨床的意義

研究概要

パーキンソン病では、ドパミン系ニューロンのほかセロトニン系ニューロンの変性も報告されている。我々はパーキンソン病患者の髄液セロトニンおよびその関連物質を測定しドパミン系物質の変化、パーキンソン病の重症度、認知機能の低下などとの関係を検討した。
1)未治療のパーキンソン病患者では、総(有意型および抱合型)セロトニン、5-hydroxytryptophan,kynurenineおよび3-hydroxykynurenineの濃度が対照例と比較してパーキンソン病患者で有意に減少していた。総セロトニン濃度の減少はHoehn and Yahrの重症度(-0.74、p=0.004)、固縮(r=-0.67、p=0.010)、無動(r=-0.65、r=0.013)、すくみ足(r=-0.72、r=0.005)、の重症度と有意の負の相関が認められ、すくみ足との相関が特に高かった。
2)未治療のパーキンソン病患者では髄液ドパミン濃度も低下しており、セトニン濃度と有意な正の相関を示した。対照例では、むしろ負の相関傾向を示した。
3)L-dopa治療中の患者ではドパミン濃度が著しく上昇し、セロトニン濃度は、未治療例よりも更に低下していた。トリプトファン、5-HIAA、3-hydroxykynurenineはL-dopa投与と正の相関を示した。セロトニン濃度の低下は、痴呆スケールの低下と有意の相関を示した(r=0.93、p<0.0001)。
