固形がん免疫療法の効果増強に関する研究

研究概要	ヒト末梢血単核球にin vitroでOKー432 0.1KEを貪食させ、上清中のcytokineをEIA法で測定したところ、TNF、ILー1α、ILー1βは12ー24時間後に最大値となることが確認された。MethA腫瘍皮下移植BALB/cマウスを用い、fibrinogen 16mg(XIII因子含有)とOKー432 0.2KEをaprotinin 0.2mlで溶解した液を腫瘍内注入して、腫瘍径の変化とその生存期間を観察するとともに注入部組織像の経時的変化を観察した。その結果、OK単独投与群に比しfibrinogen混合投与群で有意の腫瘍増殖抑制効果(p<0.05)がみられ、生存期間も著明に延長した(p<0.05)。今年度に購入したpersonal computerはこの統計処理に使用している。組織学的検討では、局注部に形成されたfibrin meshwork内にOKー432粒子が長期にわたり残留し、OKー432を貪食した細胞(Mφ)やリンパ球・形質細胞・好酸球が多数集積するとともに、広範な腫瘍壊死が観察された。OKー432 5KE/fibrinogen 80mgの混合液をヒト大腸がん50例に対して経内視鏡的投与した.この投与には当該研究費で購入したファイバ-スコ-プが有用であった.局注後数日でがん巣部の出血と周堤隆起の崩壊がおこり、7日目には腫瘍の平坦化や潰瘍の縮小瘢痕化がみられた。病理組織学的な検討では、注入されたfibrinogenは局所に繊細なfibrin網を形成しており、その中に多数の好中球・Mφ・多核巨細胞・形質細胞・リンパ球が集積するとともに著しい血管新生がみられ、その周囲を囲繞するようにおびただしい数の好酸球が集合する像が見られた。経時的に変化を追うと、局注後1週間もするとがん間質は著しいリンパ球浸潤をともなう肉芽組織と化し、3週間後には強い線維化をきたして瘢痕化することが分かった。これらの一連の反応は、免疫学的には肉芽腫形成をともなう遅延型アレルギ-反応と解釈され、がんはその間質の変性にともなって退縮していた。
