インスリン受容体遺伝子上流のpromoter活性低下によるインスリン受容体異常症

研究概要	インスリン受容体異常症の9家系の遺伝子解析を行い、今日までにインスリン受容体遺伝子のcoding regionに異常と認めなかった家族を除いて6家系にmissense mutation を認めた。即ち、Ang^<735>→Sen^<735>、Ab^<1048>→Asp^<1048>、Trp^<1193>→heu^<1193>二家系、Pro^<193>→Leu^<193>とGlu^<179>→Asp^<1179>の6家系である。この中Arg^<723>→Ser^<723>及びPro^<193>→Leu^<193>の二家系では homozygote の point mutation をみとめインスリン低抗性も著しいことをみとめた。 Arg^<735>→Ser^<735>:この異常によりインスリン受容体の前駆体であるプロレセプターからの processing すなわちαとβサブユニット間でのclearageがなされず、プロレセプターのまま膜に挿入され、インスリン結合に親和性に低い受容体として存在し、このためインスリン抵抗性が招来された。 Pro^<193>→Leu^<193>:αサブユニットでのmissense mutationを示すこの異常症では膜表面にみとめられるインスリン受容体は著明に減少しており、実際、異常遺伝子をCOS7細胞にtronsfectionしたところ、膜表面への正常レセプターの挿入は障害されていた。 Trp^<1193>→Leu^<1193>:kinase domoinにみられたこのmissense mutationは二家系にみとめられたがその受容体数の減少の程度及びインスリン抵抗、acanthosis nigricansなど二家系の間で相異があった。βサブユニットの変異であったが、インスリン受容体数が減少するという興味ある結果を得、更に詳細な検討が計画されている。 Ala^<1048>→Asp^<1048>:キナーゼドメインのmissense mutotionであるこの異常症ではATP結合は障害されず、phosphoryl transferの障害がキナーゼ低下の原因として考えられた。coding regionの正常のものは、promoterの異常の他にpost-receptor、特にIRS-1やΠ_3 leinaseの異常を検討中。
