インスリン受容体遺伝子上流のpromoter活性低下によるインスリン受容体異常症

研究概要

インスリン受容体異常症の9家系の遺伝子解析を行い、6家系にcoding regionの異常、すなわち、Arg^<735>→Ser^<735>、Pro^<193>→Leu^<193>、Ala^<1048>→Asp^<1048>、Glu^<1179>→Asp^<1179>、Trp^<1193>→Leu^<1193>が明らかとなり、それぞれの異常受容体についての特性や異常のメカニズムについてほぼ明らかにされた。すなわち、Ala^<1048>→Asp^<1048>のβサブユニットのチロシンキナーゼ活性の低下によるものを除いて、他の5家系ではインスリン結合の低下であった。この中、Arg^<735>→Ser^<735>は結合親和性の低下であったが、他のものはインスリン受容体そのものの数の減少であり、生合成過程での障害のため分解され細胞膜に挿入されないことが明らかとなった。Glu^<1179>→Asp^<1179>、Trp^<1193>→Leu^<1193>は異常遺伝子をCOS7細胞にtransfectionすると大部分の異常受容体は生合成過程で分解されるが一部は膜に挿入される。これらの膜の受容体のチロシンキナーゼ活性は著しく低下していた。このようにβサブユニットのチロシンキナーゼドメインの異常により、生合成過程で分解されるということは現在まで報告されていない。
これら6家系以外にインスリン受容体数の減少と、受容体mRNAの減少を示す1家系の患者のpromoter領域を検討するとG C boxにある-603の位置のnucleotideに異常を認めたが、これをヒト成長ホルモン遺伝子をリポーター遺伝子として組み込み成長ホルモンの産生される濃度をmouse L細胞の上清にて測定したが特に減少は認められなかった。したがって、この部位以外の未知のregulatory regionの異常によるものと考えられる。
