研究概要

我々は、臓器特異性自己免疫疾患の代仏であるバセドウ病と橋本病の成因を明らかにするために、まず自己免疫性甲状腺疾患患者甲状腺組織内での各種サイトカイン・接着因子の役割を明らかにした。すなはち、甲状腺細胞はHLA-Iと-IIを発現し、血管新生と単核球の浸潤を伴っているが、さらに接着因子であるICAM-Iも強く発現していることを明らかにした。in vitroでは、HLA-I発現はインターフェロン-α、β、γ、腫瘍壊死因子-αで誘導され、HLA-II発現はインターフェロン-γで刺激される。ICAM-1はインターロイキン-1、インターフェロン-γ、腫瘍壊死因子-αでその発現が促進される。甲状腺内浸潤リンパ球では活性化されたヘルパー・記憶T細胞が増加し、各種接着因子(LFA-1、CD2、VLA-4、VLA-5)の発現も亢進している。甲状腺内血管内皮細胞においても接着因子(ICAM-I)の発現が増加している。これらの事実は、末梢血中を循環しているリンパ球が甲状腺内に浸潤し、記憶T細胞が甲状腺内に残存し、甲状腺内での甲状腺細胞や内皮細胞との細胞間相互作用を維持し、自己免疫反応を持続させることを強く示唆している。このような自己免疫反応において自己抗原がどの様に関わっているか興味が持たれるが、我々は、甲状腺特異蛋白の一つであるTSH受容体でその抗原性に関する研究を行い、TSH受容体のgenomeにおいて制限酵素Taq 1で多型性を見出した。これがTSH受容体蛋白の異常に関連していてこのためにこの蛋白が自己抗原になっている可能性が示唆される。さらに我々は、4kbのTSH受容体cDNAの他に1kbの短いcDNAをクローニングした。このことは、TSH受容体が約100kdの蛋白として甲状腺細胞に発現されているのみでなく、約45kdの分泌型蛋白として血中に放出されている可能性を示唆するものであり、血中のTSH受容体蛋白も自己抗原として自己免疫反応に関与しているかもしれない。
