研究概要

1.肢端黒子型黒色腫の3例目を単回照射で治癒させ得た。リンパ節転移巣をもつ黒色腫症例にL-^<18>F^<10>BPA投与によるPET(positron emission tomography)を試行した。深在黒色腫をも特異的に検知できる新診断法として有望。2〜3.中性子を生体深部に到達させるため、(1)病巣前方にピンポン球を置き空隙を作成、(2)体液の一部を重水置換、(3)中性子線束は平行成分と等方成分を持つが、前者の比を増加させる、の3手法を追求。各々をファントム実験と輸送計算で定量評価し、よい一致をみた。(3)の実用化のためにコリメータを改造。4.照射場中の熱外中性子成分の制御・調製を目的に数値計算を施行。炉心・照射場間に挿入するスペクトラムシフターの種類と厚さの変更でエネルギースペクトルとその強度を自由に変え得ること判明。5.(1)メチレンブルー法による迅速定量で1.記載症例の血中 ^<10>B濃度を所望時間内に求め得た。(2)血中p- ^<10>BPAのHPLCによる定量、(3)m- ^<10>BPAの合成と光学分割は予期通りに成功。6. ^<10>BPA点滴静注患者の腫瘍・血液・正常皮膚内 ^<10>B濃度の経時変化を測定、clearanceの皮膚/血液、腫瘍/血液比間に相関性を認めた。これに基づき点滴直後の血中 ^<10>B濃度から皮内 ^<10>B濃度の経時的変化を推定し、治療計画を定めた。3例中2例では所期の腫瘍褪縮が、許容範囲内の皮膚障害で得られた。1例は浅い潰瘍後治癒。7.X線感受性と不感受性の黒色腫細胞に ^<10>B-BPA投与、熱中性子照射。両者のDoに差なし。本処理を受けた細胞は障害修復不可能。8. ^<10>BPA単独および利尿剤Lasix同時静注の黒色腫担癌ハムスター2群で ^<10>B濃度の時間推移を比較。群間に有意差なく、利尿剤による腫瘍/皮膚の ^<10>B濃度比向上には改良研究要。9.1.記載患者の照射中および後の全身管理施行。経過順調で特記事項なし。10.L- ^<18>F ^<10>BPA投与の黒色腫担癌ハムスターで腫瘍の画像診断、腫瘍内化合物濃度とその時間推移測定の可能性を立証。脳、リンパ節転移の黒色腫診断への応用に成功(1.参照)。
