圧反射の心室と動脈結合のエネルギ-効率におよぼす影響の研究

研究概要

初年次は以下のような研究を行った。
〈目的〉圧反射が心室の収縮効率に及ぼす影響を検討した。
方法〉7頭の麻酔開胸犬の大動脈起始部を剥離し電磁血流計を装着した。左室および動脈内にカテ先血圧計を挿入した。両側の頚動脈洞を単離し頚動脈洞圧を制御するための容積サ-ボポンプに接続した。頚動脈洞圧を不規則に変化させ動脈圧、左室圧を測定した。得られた左室圧の等容収縮部より等容収縮の最大左室圧を推定し、血流計で得られた一回駆出量から収縮末期エラスタンスを一心拍毎に推定した。同様に実効動脈エラスタンスを収縮末期圧と一回駆出量の比で一心拍毎に推定した。頚動脈洞圧を入力、収縮末期エラスタンスおよび実効動脈エラスタンスを出力とみなしそれぞれの間の伝達性特を算出した。得られた伝達特性に基づき心室と動脈の結合の効率について検討した。
〈結果〉左室と動脈のエラスタンスの平均値は有意な差を認めなかった。頚動脈動圧から収縮末期エラスタンスまでの伝達特性および頚動脈圧から実効動脈エラスタンスまでの伝達特性の何れも二次遅れの性質を示し、遮断周波数にも有意さは無く平均で0.02H_2であった。
〈考察〉頚動脈洞圧受容体を介しての圧反射で惹起される収縮性の変化と心拍数や血管抵抗の変化(実効動脈エラスタンス)は静的のみならず動的にもバランスしていた。その結果、両者は常に同様に変化する事から収縮末期エラスタンスと実効動脈エラスタンスの比は過渡的にも一定に保たれる。コントロ-ル状態における心室と動脈の結合効率は高く維持されていることが知られているが、本研究より心室と動脈の結合効率は圧反射がおきてる過渡的な状態においても良好に保たれることが示された。
