胃・十二指腸の適応性反応におけるカプサイシン感受性神経の役割

研究概要

カプサイシンにより一次求心性知覚神経を選択的に刺激、或いは麻痺させることにより、胃・十二指腸の適応性反応における知覚神経の役割について以下の知見を得た。
1.カプサイシンは胃内投与により塩酸エタノ-ル胃損傷の発生に対し有意な粘膜保護作用を示した。この作用はカプサイシン感受性知覚神経(Caー神経)の麻痺により著明に減弱し、またインドメタシン(IM)の前処置によっても有意に拮抗された。
2.カプサイシンの胃内適用により、酸分泌は影響されなかったが、粘膜血流及びアルカリ分泌は増大し胃運動は抑制された。これらの作用も知覚神経麻痺、或しはIMにより抑制された。
3.1M NACl及び0.2N HClなどのマイルドイリタントの胃内適用は、胃粘膜電位差(PD)の低下と共に、胃粘膜血流の増大及び胃内pHの上昇を惹起するが、Caー神経の機能的麻痺は粘膜血流の増大反応のみを有意に抑制した。一方、IMは血流増大以外にpHの上昇反応も抑制した。
4.十二指腸アルカリ分泌は粘膜の酸性化(10mM HClの10分間適用)により著明に促進するが、この反応はCaー神経の機能的麻痺により有意に阻害された。また、カプサイシンの十二指腸内適用は濃度依存的にアルカリ分泌を上昇させたが、この作用もCaー神経の機能的麻痺、及びIMの前処置により有意に抑制された。
以上の結果より、Caー神経は胃・十二指腸の適応性反応の調節機序に内因性PGと共に関与していることが判明した。また、これら組織における適応性反応は異なる複数の機序により調節されており、中でもCaー神経は血流,及びアルカリ分泌において重要であり、またその調節経路にも内因性PGが関与していることが示唆された。
