中性子捕捉療法の改善と適応拡大のための基礎的臨床的研究

研究概要

平成4年度までに9例の悪性脳腫瘍患者に対し本療法を施行、現在3名が生存している。これらの症例を詳細に検討した結果、本療法は脳室転移の無い表在性の初発例には最も良い適応となり得るが深在性のものに対してはより腫瘍集積性の強い硼素あるいはガドリニウム化合物の開発が不可不と考えられた。成果は平成4年10月第51回脳神経外科学会総会シンポジウム、同11月第5回中性子捕捉療法国際シンポジウム等で報告された。一方基礎研究面では新しい化合物の開発を目指し、現在までに核酸誘導体、各種モノクローナル抗体、ガドリニウム化合物およびアミノ酸誘導体などがスクリーニングされてきており、取り分けチミン誘導体(5'-carboranyluridinc)、ガドリニウム-硼素複合体(Gd-DTPAcarboranc)、フェニールアラニン誘導体(Boro-mcthylglycylphcnylalaninc)などに実験的脳腫瘍で高いNCT効果を認め、それらの毒性の低減化や集積性向上のためのchcmicaltuningを通し臨床応用に向けて研究を展開中である。さらに平行してDNA分子とアルキル化により結合するアジリジン誘導体など新しい化合物の合成も成しえており、現在これらのNCT効果についても実験的腫瘍系で検討中である。これらの成果は平成4年11月第5回中性子捕捉療法国際シンポジウム、平成5年3月原子力先端研究国際シンポジウムなどで発開された。炉物理用では新しい中性子コリメーターを開発し、熱中性子線の組織透過性を向上しえた。さらに脳表での中性子モニター精度が一段と向上出来た等、照射技術面での改善が得られた。これらの成果は平成4年11月中性子捕捉療法国際シンポジウム、平成5年3月京都大学原子炉実験所学術講演会などで報告された。その他正常脳に対する耐容線量評価、重水投与による腫瘍線量の改善のための研究などが進行中であり、臨床と基礎研究の両面で多くの成果が得られている。
