心・血管の構築変化と心不全の発現を規定する適応臨界点に関する基礎的研究

研究概要	I小動物心室容量連続測定システムの確立:左心室の構築変化や負荷に伴う心ポンプ機能の変化は心室内圧と心室容積の同時計測を行い、圧ー容積関係から心室エラスタンスを求めることによって定量化される。血圧負荷、容量負荷、心筋梗塞による構築変化は家兎程度の小動物に作り易いので、基礎的研究は小動物が頻用されている。しかし、小動物の心室容積を連続的に測定する方法が無かったので、心ポンプ機能の変化についての詳細な研究は行い得なかった。本研究では2電極間のコンダクタンスの変化が導電体の容積に比例して変化する事に着目し、2極コンダクタンス計測システムを開発した。本システムは家兎心室容積の変化を時間遅れなく連続測定する事が可能であった。カテ先トランスデューサーによる心室内圧をコンダクタンスと同時計測すると圧ー容積関係のリサージュを描く事ができたので、心室エラスタンスの計測が可能となった。 II力学的負荷による心・血管構築変化家兎を麻酔し、片腎摘出と対側腎上極の腎動脈を結紮する事によって腎血管性高血圧を作製し、左心室圧負荷モデルを作製した。また、腹部大動脈と下大静脈を側々吻合し、静脈還流量増加による右心室圧負荷モデルを作製した。負荷の程度とその持続時間が心不全の発生にどの様に関連するかを検討中である。 III左右心室の相互干渉作用の検討成犬の心臓を麻酔人工呼吸下に摘出し、交叉潅流心モデルを作製し全血で潅流した。右心室と左心室の内腔に風船を挿入し、両心室内腔の容積を任意のレベルに変え、心室内腔の血圧は風船内に留置した高感度カテ先血圧計で計測し、等容収縮時の心室収縮性(Ees)と拡張特性(Ed)を求めた。左→右への相互干渉が有意に高かった。
