A-Cバイパス用自家組織播種型人工血管開発に関する基礎的研究

研究概要	宿主の治癒力を賦活化し、人工血管開存性向上に活用するため、我々は自家組織を細切して合成高分子材料製の管に播種し、内膜治癒を促進さることに成功した。自家組織としては、末梢静脈組織、大網組織、骨髄組織、皮下脂肪組織等を利用した。末梢静脈組織は血管壁構築細胞をすべて含むため、移植片に適しているが採取量に限りがある。それでも動物実験的にはこの方法で400日の長期開存成績を得た。大網組織には組織治癒を促進させる作用があり、我々もin vitroの細胞培養系にてそれを確認するとともに、in vivoで内膜治癒促進効果を確認した。皮下脂肪組織は入手が容易であるため、現在臨床応用にまで研究を進めることができた。そして長期に小口径人工血管を開存させることに成功している。骨髄組織は幼若な細胞を多量に含むことから、その分裂能、分化能を利用することができる。移植された細胞は生着のための栄養の要求、血液細胞を作るための素材の要求、造血したのちの血球を送り出すための経路の要求などで強い血管新生作用を発揮する。そのため、骨髄組織を移動した人工血管には植え込み直後よりbFGFという細胞増殖因子が検出できる。そして無数の毛細血管新生がみられ、人工血管内膜治癒が急速に得られた。このような現象はこれまで世界中でだれも認めることのできなかったことであるが小口径人工血管開存製維持には最適と思われる。以上の結果、このような作用を活用した人工血管が天然の抗血栓性を経て永久的に開存することができるため、A-Cバイパス用人工血管としての活用が期待される。
