癌関連遺伝子の発現からみた食道癌発生並びに進展形式に関する研究

研究概要

食道癌にはいわゆる食道異型上皮を伴う症例が多く認められるが、その前癌病変としての意義について一定の見解は得られていない。本年度は、食道癌組織に並存する食道異型上皮を対象に、癌抑制遺伝子p53の変異産物である異常p53蛋白の発現を検索することにより、食道異型上皮の意義を明らかにすることを目的として検討を行った。術前非治療の食道癌切除症例のうち、食道異型上皮を並存する35症例、異型上皮179病変を対象とし、抗p53抗体pAb1801による免疫組織染色を施行した。食道異型上皮179病変中、92病変(51.4%)に異常p53蛋白の発現が認められた。これらを、異型細胞が上皮内に占める割合によってその異型度を定義したWHOの分類に従ってmild、moderate、severeの三段階にわけて検討したが、異型度とp53異常蛋白の陽性率には相関はなかった。次に主病巣において癌細胞がp53蛋白発現陽性であった112例を対象に、異型上皮と主病巣との連続性の有無をp53異常蛋白の発現について検討を行った。この結果、主病巣に連続する異型上皮においてはその94.8%において異型細胞層にp53蛋白の発現を認めた。これに対して、主病巣に連続しない異型上皮においてはその発現陽性率は44%に過ぎなかった。以上の結果より、食道異型上皮は、癌抑制遺伝子p53の変異からみると、その異型細胞層では既に癌に準じた変化を来たしており、食道異型上皮は潜在癌病変であるという概念を支持するものと考えられた。
