酸素中毒肺の予防に関する研究

研究概要	成塾ウサギを100%酸素化で24時間人工換気(最高気道内圧:35cmH_2O)を行い酸素中毒肺モデルを作製した。酸素中毒肺の発症には好中球から産生される活性酸素や血小板から放出される種々のケミカルメディエーターが重要な役割を果していると考えられる。一酸化窒素(NO)は平滑筋弛緩作用のみならず好中球及び血小板の凝集、接着の抑制作用をもつと報告されている。このモデルを使用しNOの酸素中毒肺予防効果について検討した。NOは人工換気開始とともに40ppmを24時間投与した。検討項目は経時的PaO_2、末梢白血球数、血小板数、乾湿重量比(W/D ratio)、肺組織(光顕)、気道洗浄液(BALF)中の好中球の全細胞数に占める割合(%PMN)、血中メトヘモグロビン(MetHb)、血漿nitrite+nitrateとした。NO投与群とNO非投与群とでPaO_2、末梢白血球数、血小板数、W/D ratio、BALF中の%PMNに有意差は見られなかった。肺組織(光顕)でもNO投与群とNO非投与群ともに炎症細胞浸潤や肺出血、肺胞壁の肥厚が観察された。血中MetHb,血漿nitrite+nitrateはNO投与群で上昇していた。呼気のNO_2濃度は0.5ppm程度にとどまりNO投与群におけるNO_2の副作用は考えにくかった。この実験ではNO投与群とNO非投与群との間に差が得られなかったが、より重篤な肺障害における効果を調べる必要がある。
