天疱瘡の発症機序の解明と新しい診断、治療法の開発

研究概要	本年度は、130-KD尋常性天疱瘡抗原ならびに160-KD落葉状天疱瘡抗原の正常ヒト表皮における量的分布状態を明らかにし、それぞれ病態の発症に重要な役割を果たしている表皮カドヘリンが正常皮膚の接着にどのレベルで関与しているのかを検討した。すなわち昨年までの研究により、両抗原は表皮デスモゾームにのみ限局して発現されていることが明らかとなっていた。本年度は表皮の異なったレベルにおける両抗原のデスモゾームにおける分布状態に有意な差があるか否か、またどのような差があるかについて研究した。具体的には複数の正常人皮膚を、本研究費の補助により導入したcryoultramicrotomyによる免疫電顕の基質とし、患者からえた精製自己抗体と反応させ、表皮顆粒層、有棘層、基底細胞層に存在する各デスモゾームをラベルする金コロイド粒子を定量的に測定、統計学的に処理した。その結果、尋常性天疱瘡抗原は表皮上層にくらべ表皮下層の基底細胞層デスモゾームに有意に量的に豊富に発現していた。逆に落葉状天疱瘡抗原は表皮上層のデスモゾームにおいて、表皮下層のそれより有意に量的に豊富に発現されていた。以上の結果から、尋常性天疱瘡と落葉性天疱瘡にみられるacantholysisにもとづく水疱の形成部位が両者で異なる原因は、その自己抗体が認識する自己抗原の量的分布状態がヒト表皮のレベルにより異なっているためであることが明らかとなった。またこれら自己抗体の患者皮膚表皮の各レベルにおけるデスモゾームへの結合を阻害することができれば、水疱の形成を完全に抑制することができる可能性も示唆された。
