皮膚刺激による感覚神経と皮質応答の非侵襲的計測-電位・磁界と機能的MRIの統合

研究概要

1.末梢神経活動の3次元的計測と定量化
末梢神経活動の多点磁界計測により、動く4重極子(脳分極電流と再分極電流)の詳細な時間的空間的分布を可視化した。更に第2指指神経を刺激したときの上肢からの神経磁界と手関節部の正中神経を刺激したときの磁界と比較するとその信号強度は組織学的に計測された各々の神経に含まれる線維数を忠実に反映することを見出した。これらの知見は末梢神経病変の高精度な非侵襲的機能診断の可能性を示している。末梢神経障害の少数例についての予備実験では磁界計測の診断精度の高いことが明らかになった。
2.電位計測と磁界計測の統合による脳内信号源の推定
電位計測と磁界計測は各々長所と短所を合わせ持っている。各々の持つ情報をうまく組み合わせて信号解析を行うことにより、聴覚誘発電位の Na 成分が従来考えられていた単一成分ではなく皮質成分と皮質下成分に明確に区別できることを初めて明らかにした。
3.体性感覚誘発磁界に含まれる高周波信号の生理学的意義
体性感覚誘発磁界の1次応答 N20m に重畳して記録される 300-800 Hzの高周波信号が視床皮質求心線維を受ける皮質4層に存在する抑制ニューロン活動を反映することを明らかにした。この新しい知見は基礎的ならびに臨床的脳科学にとり以下の点で重要である。
a.従来の脳波・脳磁界は興奮性錐体細胞の活動を反映し、抑制系のニューロン活動については注目されていなかった。
b.ヒトの知覚・認知・運動・記録・学習あるいは皮質の可塑性に重要な働きをする抑制系ニューロンの活動を捉えることができれば、この信号を指標としてこれらの脳高次機能の解明に道を開くことができる。
c.全く非侵襲的計測である。
