神経成長因子関連薬物のアルツハイマー型老年期痴呆薬としての可能性について

研究概要	神経成長因子(NGF)は、大脳基底核のコリン作動性神経の生存維持に、重要な役割を果たしている。一方、アルツハイマー型老年期痴呆(AD)では、発症初期段階より上記神経系の損傷が顕著である。OlsonらはNGFをAD患者の脳室内へ3ヶ月間持続的に注入することにより患者の大脳皮質のニコチン受容体および脳血流量が増加し、学習記憶障害が改善することを報告している。これらの知見からNGFによりADを治療できる可能性があるが、NGFを末梢投与すると、NGFはペプチドであるので血液脳関門を通過しにくく、蛋白分解酵素により分解される可能性がある。従って、NGFそのものを経口投与するためには、ドラッグデリバリーシステムを開発する必要がある。そこで、Olsonらは患者の脳内へ直接NGFを投与することを試みたが、この方法は、手術を必要とし簡便でないうえに、患者のQuality of lifeの上からも問題がある。これらの欠点をクリアする目的で、我々は、経口投与が可能でNGF様活性を持つ薬物の開発を試みた。in vitroでNGF様活性を示したプロペントフィリン、イデベノン、およびそのトリメチルキノン誘導体に注目して、これらを経口投与した。その結果これらの薬物は、in vivoで脳内のNGFmRNAを発現し、NGF含量を増加し、アセチルコリン合成酵素の活性を増加した。興味あることに、これらの作用は脳損傷動物および老齢動物のみに認められ、正常成熟動物では認められなかった。上記薬物は、脳損傷動物の場所に対する馴れ、水迷路および受動的回避反応課題における学習記憶障害を改善した。以上の結果から、これらの薬物は、ADのような痴呆の初期診断が可能となれば治療薬として有用になる可能性があると思われる。
