先天性腎性尿崩症におけるバゾプレシンV2受容体の遺伝子解析-障害部位の確定と臓器特異性に関する検討

研究概要	先天性腎性尿崩症例につき、腎尿細管のV2受容体系の障害部位の検討および遺伝子解析を行った。症例1,2の伝達様式は常染色体優性と考えられた。また後述するcAMPの反応性の検討からも、腎性尿崩症の中で過半を占める1型ではなく2型と考えられた。対照として4例の先天性腎性尿崩症1型の症例についても、病変部位の確認を行った。尿中cyclic AMP濃度は、ラジオイムノアッセイにて、血液凝固第8因子は発色性APTT法にて、von Willebrand因子活性は固定血小板凝集法にて測定した。DNAの抽出はJeanpierre(1987)により報告された方法を改変しておこなった。HindIII,BamHIにて消化したDNAに対し、Lolaitらの方法も参考にしつつ、Southern blot法にて検討を加えた。32-Pで標識されたヒトV2受容体cDNAの428bp fragmentをprobeとした。PCRにおけるプライマーはV2受容体の4-6domainをコードする332bp fragmentに対するものである。症例1においては、DDAVP投与後に尿中cAMP濃度は609%であり、症例2では前値の1984%まで増加し、120分後には96%ほぼ前値に類似した値にまで再度低下した。これらの結果はこの2症例において、protein kinase Aやwater channelの異常が存在している可能性を考えさせる結果であった。対照の4例においてはこれと異なり、78%-126%と有意の上昇を示さず、遺伝形式も伴性劣性であり、DDAVP投与に対する凝固因子の増加反応が不十分な例もあった。症例1、2について遺伝子解析を試みたが、既知のV2受容体遺伝子の異常は存在しないことが確認された。このことから本症2型においてはV2受容体系の後半部に障害部位があることが示唆された。それが最近クローニングされたaquaporin-2ないしそれ以外の異常であるかの研究は、腎性尿崩症の病因解明のみならず、aquaporin-2を含む腎集合管の水濃縮機構をより明らかにするためにも、重要な意義のあるものと考えられる。
