皮膚癌による基底膜構造の破壊と予後との相関

研究概要	皮膚癌による皮膚基底膜構造の破壊や細胞生物学的変化と予後との相関関係を解明することは、皮膚科の臨床において極めて重要である。本研究の目的は、この問題点を解決することにあった。実際には、基底膜の微細な構造変化を電顕的に観察する従来までの方法に加え、最近開発された複数の皮膚基底膜抗原タンパクに対するモノクローナル抗体を用いて、本研究代表者らが開発した新しい免疫電顕、並びに免疫組織染色の手法を応用した。本研究では、採取した皮膚癌の組織形態の保持と、タンパクの抗原性の保持という両者の条件を同時に満足させ、同一組織切片上で皮膚癌による基底膜構造の破壊と予後との相関を解明した。具体的には、皮膚癌の形態を、化学固定により保持させる方法では、多くの重要な皮膚基底膜タンパクの抗原性を失活させてしまうので、これを避け皮膚組織の形態と抗原性の両者を同時に保持しうるよう、液体プロパン(-190℃)を凍結源として皮膚を急速凍結固定した。その後、光顕用には組織を、-60℃で凍結乾燥させた。免疫電顕用には-80℃にてメタノール中にて凍結置換により組織中の水分を低温のまま完全に除去した。これら組織を基質として皮膚基底膜の形態、免疫組織学的変化を基底膜抗原タンパクに対する種々のモノクローナル抗体を用いて検索した。その結果、皮膚癌の早期浸潤過程では、4型コラゲン、ラミニンがまず抗原性を低下させ、7型コラゲン、GB3抗原、BP抗原は比較的早期の皮膚癌浸潤過程では抗原性を保持していることが明らかとなった。
