甲状腺内リンパ球とサイトカインによるin vitroでのバセドウ病の再現

研究概要

バセドウ病は、TSH receptor刺激性の自己抗体(TSI)が産生されるために生じる甲状腺ホルモン分泌過剰症であるが、バセドウ病が悪化する免疫学的な機序は全く不明である。我々はバセドウ病患者より得られた甲状腺濾胞と甲状腺より採取されたリンパ球を共に培養しておき(coculture系)、TSHに反応して、^<125>Iを取り込み、甲状腺ホルモン(^<125>I-T_4および^<125>I-T_3)を分泌する系を確立している(文献1-3)。そこで、このcoculture系にIgGの産生を促進するサイトカイン(IL-4,IL-6,IL-10)を添加し、甲状腺ホルモンの合成および分泌に及ぼす影響を検討した。
IL-4はcoculture系でTSHによって誘発される甲状腺機能(^<125>Iの甲状腺濾胞への取り込み、および^<125>I-T_4の分泌)を濃度依存性に促進した。しかし、TSH非存在下では甲状腺機能を促進できなかった。また、IL-4はリンパ球の非存在下でも甲状腺機能を促進できたが、IL-6は甲状腺機能に抑制的に作用した(文献2)。バセドウ病の甲状腺濾胞内には、T細胞のみならず、B細胞も浸潤しているので、現在、IL-4の甲状腺ホルモン分泌刺激作用が、Bリンパ球を介するものなのか、あるいは甲状腺濾胞にたいする直接作用なのかを検討中である。なお、Th1細胞に作用して、IL-2やinterferon-gammaの産生を抑制するIL-10も甲状腺機能に促進的に作用した。従って,バセドウ病では、このようなcytokine networkにより甲状腺機能が調節されていることが明らかになった。
この研究は米国の甲状腺学会でも大変、注目されており、今後、抗IgG抗体やバセドウ病でないヒト甲状腺を用いて、IL-4の甲状腺ホルモン促進作用機序を解明して行く予定である。なお、この研究中に、最近C型肝炎に広範に使用されているIFN-alphaやIFN-betaはヒト甲状腺機能を一過性に抑制することを見いだした(文献3)。
