小腸移植後の消化吸収能と消化管ホルモンの動態

研究概要

今年度短腸症候群に対する部分小腸移植の効果を遺伝子背景の明らかなラットを用いて検討した。まず短腸症候群の病態ならびに発育に必要な移植腸管の長さを検討し、次に消化吸収能等の栄養学的面から空腸あるいは回腸いずれを移植すべきかを検討した。1.全小腸摘除を行ない、ラットにおける短腸症候群の病態を検討した。生後2-4週の若年ラットでは小腸を全摘すると静脈栄養等の補助なしに全て1週間以内に死亡するが、生後8週間以降のラットでは低栄養状態を持続しながらも長期生存した。腸管切除により長期低栄養を示したラットは正球性低色素性貧血を呈し、免疫系臓器にも異常をきたした。特に肝臓では focal hepatitisを生じた。8週以降の短腸ラットに対して回腸を10cm移植すると正常に近い体重増加が観察され、部分小腸移植成功の可能性を示唆した。2.移植片に空腸と回腸をそれぞれ35cm利用したうえで、実験群を正常無処置群、空腸移植群、回腸移植群、短腸群(小腸全摘)に分けて検討した。いずれの群も食餌摂取量は同様であったが、短腸群は術後体重増加が著しく障害され、空腸移植群、回腸移植群ともに正常群と同様の体重増加を示した。排便中の脂肪排泄量は測定すると、空腸移植群では脂肪吸収が障害されていたが、回腸移植群では正常群と同様の結果を示した。胆汁中の胆汁酸量を測定すると、空腸移植群では低下していたが、回腸移植群は正常群と同等であった。移植腸管内にオレイン酸負荷を行なう胆汁流出量に差はなかったが、血清リパーゼ値の反応が回腸移植群で有意に高値を示した。さらにリンパ管造影を行ない、移植後リンパ管再疎通性を検討すると、術後3日めでいまだ再疎通していないが、術後4週めには移植腸管化らレシピエントの胸管まで再疎通していることが確認された。
