ヒト成長ホルモン遺伝子の構造異常に関する研究

研究概要	GH分泌不全や痩せのない家族性低身長例で、内因性GHに対する反応不良、即ち血漿IGF-1が低値の症例を何例か経験した。外因性GHに対する反応性から臨床的にKowarski症候群(生物学的不活性型GH)やLaron症候群を鑑別し、それぞれGHおよびGH受容体遺伝子異常の解析を試みた。上記の基準を満たす3例について、まずNB2細胞の増殖活性を指標とするbioassay系でGHの生物活性(BA)を測定した。BAとIRMAによるGH値の比は対照;1.15±0.40、症例1;1.25±0.03、症例2;0.54±0.18、症例3;0.46±0.25と2例でBAの有意な低下を認めた。次に患者末梢血よりDNAを抽出、GH-1遺伝子に特異的なプライマーを用いてPCR法で増幅し、得られた断片をサブクローニング後dideoxy法でsequenceした。また複数クローンで異常があればPCR産物を直接sequence法でも確認した。症例1、2ではエクソン部分に異常を認めなかった。症例3では第2イントロンの1170と1171番目の塩基間に12塩基の挿入を認めた。エクソン部は現在確認中である。スプライス異常を考え症例2と3については末梢血リンパ球よりRNAを抽出しRT-PCRで増幅したcDNAについても解析中である。異常が確認できたら、変異GHを発現しBAの検討を予定している。一方Noonan症候群の1例で外因性に急性投与したGHに対し血漿IGF-Iの反応不良を呈したためGH受容体遺伝子を解析したところコドン561のACU(threonin)がCCU(proline)に変わるmissense変異を6クローン中3クローンに認めた。またこのPCR断片の半分が本来なら同部を切断可能なStu Iで切断できずヘテロな点突然変異と考えた。現在GH受容体遺伝子のexon部を中心に他の構造異常の有無、及び家族の検索を進めている。またこの変異の機能的意義についても異常遺伝子を発現させ、インスリン遺伝子転写活性などを指標に検討する予定である。
