研究概要	中国上海市北の海門市における肝がんに対する宿主要因(がん遺伝子・がん抑制遺伝子、アルデヒド脱水素酵素[ALDH2]及びCYP2E1遺伝子多型など)と環境要因(HBV/HCV感染、アフラトキシンB_1汚染食品摂取、当該地域のクリーク水・溜池水などの飲用[特に、藻類マイクロシスティスが産生するマイクロシスチンの摂取]、喫煙・飲酒歴など)の関与及びそれら諸要因の交互作用について、環境生態学、分子生物学、腫瘍病理学、疫学などの諸観点から総合的に調べ、肝がん発生予防に資することを目的とする。 1)海門市におけるクリーク水・溜池水などから藻類マイクロシスティス及びマイクロシスチンの分析を実施しており、現地の住民が飲用するクリーク水・溜池水中マイクロシスチンを検出した。マイクロシスチンの環境中植物連鎖を考慮して、水産食品(魚・貝・海老・蟹などの水棲生物)中のマイクロシスチン分析のシステム開発も急いでいる。 2)マイクロシスチンLRを用いた中期発がん動物実験を行い、マイクロシスチンに肝がんプロモーター作用があることを明らかにした。 3)食品(ピ-ナッツ、コーン、小麦など)を採取して、マイコトキシンの分析を試みたところ、アフラトキシンB_1に加えてフモニシンも検出された。 4)アフラトキシンB_1誘発ラット肝がんから細胞系を樹立して、神経分化誘導因子の肝がんプロモーター活性の研究を行っている。 5)現地の肝がんパラフィンブロック標本を利用して、マクロ・ミクロ病理学的研究、マイクロシスチンの検索及びがん遺伝子・がん抑制遺伝子分析を行った。肝硬変ないし繊維化を伴わない肝がんが多く、アフラトキシンB_1関連肝がんに特異的なp53遺伝子変異も観察された。 6)海門市において発生した肝がん症例群と無作為抽出した地域住民対照群の134ペアについて、飲料水の種類と飲料状況、アフラトキシンB_1汚染食品摂取状況、喫煙・飲酒歴などに関する調査票調査を行った。単変量解析及び多変量解析により、肝疾患家族歴、肝疾患既往歴などが対照群より症例群に統計学的に有意に多かった。すなわち、症例群には肝疾患の家族集積性があり、そのような家族内環境中で母児垂直感染・幼児期水平感染によってHBVキャリアとなり、その後、一定の潜伏期を経て肝がんが発生したものと考えられた。今回の調査では、クリーク水・溜池水などの飲用に関しては、症例群と対照群の間に有意な差異を認めなかった。対照群に対して症例群の収入は少なく、卵や魚の摂取頻度も少なかったが、これは現地の肝がん発生に低栄養が関連している可能性を示唆しているかも知れない。AFB_1汚染食品であるピ-ナッツ、コーン、小麦などの摂取には両群に差異を認めなかった。なお、喫煙・飲酒に関しても両群に差異はなかった。 7)肝がん症例と対照群より血液を採取し、血漿を利用してHBV血清マーカー及びHBVサブタイプ、αフェトプロテイン、セレニウムなどの微量元素・物質などの分析、アフラトキシンB_1アダクトの分析、発がんプロモーター指標としてのTNFα活性の分析を行っている。末梢リンパ球を利用した宿主要因(アルデヒド脱水素酵素[ALDH2]及びCYP2E1の遺伝子多型など)の分析を試みている。 HBsAg及びanti-HBc陽性のものが対照群より症例群に多く、現地の肝がんは主にHBV関連肝がんであることが確認された。肝発がんプロモーターであるTNFα値は対照群より症例群が高値であった。アルコール代謝関連のALDH2及びCYP2E1の遺伝子多型においては、症例群と対照群の間に有意な差異は観察されなかった。今後は、血漿の代わりに血清を収集して、HVCジェンタイプ及びHGVに関する解析も加えて研究を行う予定である。
