悪性腫瘍における樹状細胞と癌遺伝子産物発現に関する臨床病理学的研究

研究概要

本研究は、癌腫を中心としてS-100蛋白陽性樹状細胞(S-100DC)出現と腫瘍の悪性度、進展、予後との関連性を検討し、一部癌抑制遺伝子産物の内、p53について検索した。中国側の症例の検討は、症例数は少ないが、膀胱癌、咽頭癌を中心に若干の検討が可能であった。
まず日本側で得られたデータでは、皮膚の悪性腫瘍の内、偏平上皮癌、ボ-エン病・基底細胞癌について検討し、扁平上皮癌の内分化度の高い組織内でのS-100DC出現が統計的に有意に高く、基底細胞癌では数が少なく、S-100DCの形も小型であった。ボ-エン病はこの両者の中間的性格を有していた。ついで肝細胞癌については、S-100DC出現は54例中40例に認められ、数の多いものの予後がより良好であり、又、腫瘍の増大とS-100DCの数は密接な関連を有しており、肝癌おいてはS-100DCは予後判定の有用なマーカーであることを明らかにした。膀胱癌でも同様の結果が得られた。又、食道癌においても同様にS-100DCの出現数と予後は密接な相関を有していることが示された。p53については、皮脂腺癌において、転移や再発を生じたものでより高頻度に強く陽性となる所見が得られ、食道癌でもp53陽性例の予後は不良であった。このようにS-100DCは予後良好を示唆するマーカーであり、逆にp53は予後不良をあらわすマーカーであることが明らかであった。
中国側の症例について、膀胱癌5例、咽頭癌8例、咽頭乳頭癌2例について、S-100DCを検証したデータでは、やはり異型度が低いか分化度が高い症例ではS-100DCの出現が高異型度や低分化な症例に比し数も多く、形も多彩で大型のものが認められ、日本側で得られたデータと類似の傾向が得られた。
今後は更に症例を増やして、p53や他の癌遺伝子産物発現とも併せ比較検討したい。
