研究概要

前年に引き続き、12.5万人のコホート集団と2.5万人の産業企業体コホートのがん死亡と罹患状況を調査した。5年後の中間調査も本年で終了した。各地域でのコホート集団について、方法論の検討や、要因調査を行った。
結果:1993年までに4,886例が死亡、1994年度は調査は未完了であるが、1,000〜1,200例の死亡が見込まれる。死亡者の死因分布が新生物34%、循環器疾患35.1%、呼吸器疾患11.1%で80%を占める。新生物死亡は、胃22.6%、肺17.5%、肝10.5%、大腸、膵、胆道、白血病が7〜8%であった。全日本と比べ少し死亡率は低いが、がんの分布は類似している。罹病状況は約70%の地域で実施中であるが、精度に若干差があり、全体集計に入っていない。中間調査は、罹病歴、生活様式、喫煙、飲酒、がん検診についての再調査が中心で、3年間に5.5万人実施した。いろいろな項目で変動がみられたが、概観してみると、おおむね類似した傾向があった。喫煙率は男女とも15%前後低下していた。転出者は最近3年間、年間約500例で10年累積しても全体の10%を越さないと推定された。その他約2万人が新しいコホートとして追跡することとなった。各地域ではこれらのデータを用い方法論や発生要因の検討を実施している。産業企業体コホートの成績は、全死亡率は日本全体よりも低く、がん死亡率が高かった。罹病は死亡の4倍あった。血清バンクは無事保管されており、その他DNAも一部の班員で採取され保管された。委員会を設立して研究方法について検討することとなった。倫理委員会も引き続き活動し、班員間の意識調査がまとめられつつある。顧問委員からは批判、勧告があり、今後の参考とした。班としては、40〜70才の年齢層の分析を行い、その他は班員グループがよりきめの細かい検討をすることとなった。
