研究概要

(1)本班は1988〜90年に基礎調査を実施した全国50市町村の健康住民約12万人の死亡及びがん発生状況を追跡調査し、発がんに関連する要因の検証と評価を行うことを目的としている。死亡は国へ報告された死亡小票をベースに、死因・死亡年月日などの調査を毎年実施し、別にがん罹患状況を可能な地域で把握する。死因の明らかになった1994年末までの死亡者は6359名(男3702名、女2596名)である。循環器疾患が34.5%ともっとも多いものの、その約1/3は心不全である。新生物は33.6%と第2位である。男女別にみると、男の第1位は新生物であり、女では循環器疾患である。新生物死亡を臓器別に観察すると、胃7.3%、気管・気管支・肺6.0%、肝・肝内胆管3.4%、膵2.4%、胆嚢・肝外胆管2.2%の順に多い。がん罹病状況は、現在調査中であるがまだ全体の集計に入っていない。
(2)現在追跡しているコホート構成員のうちベースライン時40歳以上79歳以下のものを対象とし、死亡と喫煙・飲酒との関連を中間解析した。その結果、喫煙については全死亡、全部位がんともに喫煙、禁煙者で有意な死亡リスクの上昇が認められること、部位別には肺がん・肝臓がんで有意なリスク上昇を認め、胃がんでは喫煙状況との関連を認めないこと、飲酒については全死亡、全部位がんともに飲酒者ではリスクが低下(有意ではない)し、禁煙者でリスク上昇(有意)を認めること、部位別には、肝臓がんで禁酒者に有意なリスク上昇を認め、胃がん・肺がんでは全部位がんと同様の傾向を示すものの、有意ではないことが明らかとなった。禁酒、禁煙するものにはすでに体調不良があるために、禁酒、禁煙せざるを得なかったものが含まれている可能性があり、その背景要因を詳細に検討する必要がある。また、飲酒者でリスクの低下が見られている点につき、少量飲酒がよい影響を及ぼすのか否かを含め、量別、種類別の検討が今後必要と思われる。
