研究概要

慢性関節リウマチ(RA)を代表とする疾患群に対して、我々は慢性炎症性増殖性疾患(Chronic inflammatory proliferative disease,CIPD)なる疾患カテゴリーを提唱している。DIPDにおいては、慢性炎症の関与、免疫応答の関与、及び病的組織における細胞の慢性的増殖を特徴とする。さらに、インターロイキン6(IL-6)などのサイトカインや、ICAM-1などの種々の接着分子、あるいはc-fosなどの初期応答遺伝子などの複合的遺伝子異常発現を特徴とする。RAをCIPDのモデルとしてとらえて、RAの病態と、その病因を実験病理学の立場より、分子生物学的手段によって明らかにするのが本研究の目的である。
本研究によって以下の点を明らかにした。1)HTLV-1のp40taxのみならず、RA組織の高率に検出できるヒトパルボウイルスB19のNSI蛋白もNF-κBを介して、IL-6プロモーターを活性化することを明らかにした(東北大菅村和夫教授らとの協同研究)。2)BST-1は、未熟リンパ球に一過性に発現する。3)BST-1はマクロファージや好中球にも発現しており、レセプターとして機能する。4)BST-1欠損マウスは、免疫応答の一部に異常が観られる。5)IL-6受容体を介する細胞増殖・マクロファージ分化・あるいは、抗アポトーシスなどのCIPDの病態に深く関与すると考えられる細胞内シグナル伝達経路にJAK-STATが重要な役割を果していることを明らかにした。
