慢性関節リウマチの異なる病変関節に共通して集積しているT細胞クロノタイプの解析

研究概要

リンバ球集団において集積しているT細胞クロノタイプを検出するシステムを、RT-PCR法とSSCP法を組み合わせることで確立し、RA患者の関節病変に集積しているT細胞クローンについて検討した。RA患者末梢血には、健常人と比べると集積T細胞クローンの数は多かったが、さらに滑液リンパ球および滑膜病変局所では著明な集積が認められ、関節病変局所で抗原特異的なT細胞の免疫応答が起こっていることが判明した。さらに同じ関節の異なる病変間、および異なる関節における病変間で比較したところ、高率に同一のT細胞クローンが集積していることから、免疫応答は病変間でかなり均一であることが明らかとなった。代表的なV領域遺伝子について、これらのクローンで用いられているTCRのアミノ酸配列を比較したところ、患者間で似た配列があることが判明し、これらのことから、RAではある限定された抗原に対するT細胞の免疫応答が、その病因、病態形成に重要な役割は果たしていることが示唆された。さらに本クロノタイプ検出システムの利点を応用して、集積T細胞クローンの標的抗原の同定を試みたところ、その一部はII型コラーゲンであることが判明した。
