自己免疫性TSH受容体疾患の概念・成因および自己免疫機構の分析

研究概要	本年度研究計画として提出したもののうち1)HLA-DPの意義に関する研究は、バセドウ病、橋本病および正常甲状腺細胞を用いて、HLA-DP、-DQおよび-DPの特異抗体による染色性を検討し、自己免疫性甲状腺疾患では他臓器細胞と異り-DPの発現度が高いことを認めた。2)TSH受容体上の自己抗体のエピトープ部位の研究は、合成ペプチドを用いて甲状腺刺激抗体(TSAb)にheterogeneityが存在することを明らかにし、さらに複数ペプチドの混合実験を通じ、TSAbが受容体上の複数部位を同時に認識していることを示すとともに、受容体の立体的構造を示唆する知見もえられつつある。ペプチドに対するTSAbモノクローナル抗体も作製し、これが他部位に対しても反応すると云う極めて興味深い成績も得られつつある。甲状腺機能低下症患者末梢血Bリンパ球のクローニングから、阻害型受容体モノクローナル抗体を得、この性状の分析も進められている。3)Tリンパ球に対する免疫原性部位の研究は、バセドウ病患者末梢血から受容体ペプチドに特異的反応性を示すTリンパ球ラインを作成することに成功し、Tリンパ球受容体の解析を行って、その限定的使用がみられることを示した。免疫原性部位としては、TSH受容体の細胞外ドメインのみならず、膜貫通部位にもTリンパ球増殖刺激性があることを示し、Bリンパ球の認識抗原部位とは異ることを示唆する所見を得ている。これらの研究は、いづれも独創的なものであり、その成果に大きな期待が持たれるが、初年度の成果はなお未完成なものに留まっている。
