子宮体部の多段階発癌に関わる遺伝子群の解析とその遺伝子治療の検討

研究概要	各種婦人科腫瘍について、多段階発癌に関与する遺伝子群、癌原遺伝子、癌抑制遺伝子群の変異を検索し、全症例の約80%にこれら遺伝子に何等かの変異が存在する事を明らかにした。即ち、Ras遺伝子の点突然変異は子宮体癌の前癌病変である異型増殖症で認められ、そのほとんどがk-rasコドン12、13の点突然変異で、すべてguanineからの変異であった。発癌過程における早期の変化であることが判明した。一方、癌抑制遺伝子のp53は異型増殖症には13例中1例(8%)と少なく、癌の進展に伴いG1で2/19(11%)、G2で1/7(14%)、G3で6/14(43%)と変異率は増加し、発癌過程での比較的晩期の変化であることが示唆された。突然変異の様式もすべてがguanineからの変化でコドン248に集中していることが判明した。他の癌抑制遺伝子DCCではD18S8における制限酵素Msp-IのRFLPを利用してLOHを検討したところ、1/7(14%)に、m-RNAの低下は3/8例に認めた。Rb遺伝子のLOHも2/7(29%)に、m-RNAの低下を1/7(14%)に認めた。これらの多くは臨床進行期CIII期、あるいは未分化腺癌であった。さらに、人パピローマウイルス(HPV)がPCR法では6/47(13%)に検出でき、約5%の子宮体部癌症例においてHPVゲノムが宿主DNAに組み込まれて存在することも明らかになった。ゲノムの不安定性やDNAミスマッチ修復遺伝子とくにhMSH-2の遺伝子異常についても約20%の症例に存在する事を明らかにした。これら事実を踏まえ、p53の突然変異株に、正常p53遺伝子を含むベクターを作製し、細胞株に感染させ、p53の強制発現を促すとIn vivo及びIn vitroにおいて変異株の細胞増殖が抑制されることを見いだしている。これらの方向性にて遺伝子治療の可能性を探りつつある。
