尿素サイクル異常症の遺伝子治療に関する研究

研究概要	CAGあるいはSRαプロモーターをもつヒトOTC遺伝子をもつ2種類のアデノウイルスベクター(Adex CAGhOTC,Adex SRα OTC)を作成し、OTC欠損マウス、及びOTC欠損症のヒト由来の肝細胞を用いて、遺伝子導入実験を行った。 (1)Adex CAGhOTCとAdex SRαOTCを比較した。両者で同量のウイルスベクターが導入されたことをPCRで確認した後、肝でのOTC活性を測定した。Adex CAGhOTCを投与したOTC欠損マウスでは5%のOTC活性が41±7%まで上昇したが、Adex SRαOTCの場合は活性の上昇は得られなかった。 (2)Adex CAGhOTCでは0.5×10^9PFU以上の投与で明らかな肝OTC活性の上昇が得られた。活性の上昇はベクターの投与量に比例した。1.0×10^9PFUの投与では野生株の活性に匹敵した。肝OTC活性の上昇したマウスでは尿中オロト酸排泄量正常化した。3.5×10^9PFUを投与したマウスは死亡し肝組織で広範な壊死がみられたが、1.0×10^9PFU以下のウイルス投与での肝組織の変化は軽微であり、死亡例はなかった。 (3)Adex CAGhOTC 0.5×10^9PFUを投与したマウスについて、尿中オロト酸を経時的に測定した。投与翌日より正常化し、約60日間正常範囲内にあった。その後再度Adex CAGhOTCを投与したが、同様の効果は得られなかった。ヒト肝細胞での発現実験をOTC欠損症患者の初代培養肝細胞を用い、in vitroで行った。患者は女児で、肝移植を受けた。exonlの一塩基置換によりストップコドンを生じていた。Adex CAGhOTC,Adex SRα hOTCの両者でOTC活性の上昇は得られたが、Adex CAGhOTCで得られた活性はAdex SRα hOTCでの活性の10〜40倍高く、MOI 10で正常肝細胞と同等の活性であった。
