悪性リンパ腫の臨床病理学的、分子病理学的および生物学的研究

研究概要

前年からの研究を継続しマントル細胞リンパ腫(B細胞性)の解析を進めると共に、平成7年度に新たに節外性の鼻咽頭領域T/NK細胞リンパ腫を中心とした解析を試みた。マントル細胞リンパ腫に高率に認められるCyclin Dlの過剰発現が抗cyclinDl抗体を用いた免疫組織学的手法により検出し得ることを明らかにした。これを基盤として非ホジキンリンパ腫400例以上について抗Cyclin D1抗体による染色所見を検討した結果、分子病理学的腫瘍単位と見做し得るCyclin D1過剰発現リンパ腫36例を見出した。その多くがマントル細胞リンパ腫に相当し5年生存率11%と極めて予後不良であった。現在、それらの生物学的特性に関する本邦での確定的データの樹立を目指し、全国主要施設からCyclin Dl過剰発現リンパ腫100例以上の集積とその解析に向けて検討を進めつつある。また、鼻咽頭領域T/NK細胞リンパ腫31例についてEpstein-Barr virus(EBV)の関与を含め臨床病理学的、分子生物学的に検討を加えた。対照としてEBVを検索した約300例以上の非ホジキンリンパ腫と比較し、極めて高率なEBVの関与(84%)が認められた。その特異な臨床病態と併せT/NK細胞腫瘍全体の中で特異な位置を占めることが明らかとなった。小数ながらEBVの関与しないblastic/monomorphic NK-cell lymphomaに相当する腫瘍も含まれ、研究を継続中である。さらに、研究の過程で悪性リンパ腫そのものからその周辺に位置する境界領域的リンパ増殖病変の疾病単位としての本態の解明との必要性が痛感され、今後これらも対象として研究を展開する予定である。
