急性膵炎発症におけるカルシウムなどの細胞内情報伝達系の役割に関する研究

研究概要	1 膵炎モデルにおける検討 (1)マウスCDE(choline-deficient ethionine-supplemented diet)膵炎では、CDE摂取24時間後、48時間後とも、CCK-8の刺激による膵腺房細胞のアミラーゼ分泌能は低下していたのに対し、細胞内カルシウム動態は影響が少なかった。即ちCCK10pMではoscillation、CCK100pMでは急峻な上昇と漸減というパターンを有していた。CCK100pM投与時の細胞内カルシウムのピーク値はCDE48時間では正常より低下していたが、有意差はなかった。またoscillationの周期、振幅にも差がみられなかった。CDE膵炎における刺激分泌連関の抑制は、細胞内カルシウム以降(以外)の経路が主な原因であると示唆された。ラット膵管結紮膵炎では、膵管結紮により、閉塞開放後もCCK-8刺激による膵外分泌機能は著明に抑制されていた。膵管閉塞ラットより分離した膵腺房細胞を用いた研究では、CCK-8のみでなくCalcium ionophore A23187、およびPMA刺激によるアミラーゼ分泌も著明に抑制され、膵管閉塞による膵外分泌機能障害は細胞内カルシウムの上昇、protein kinase C機構よりもさらに分泌過程の先の部分の障害と考えられた。 2 Humanにおける検討膵頭十二指腸切除後の患者の膵管チューブからのアミラーゼ分泌は、手術前の膵管の閉塞あるいは狭窄により大きな影響を受け、閉塞を認めた患者ではアミラーゼ分泌は正常の患者の約1/10程度まで減少していた。Humanにおける分泌障害の機序については今後さらに検討する予定である。
