ヒトの歩行運動の中枢制御機序とその病態生理の解明

研究概要

歩行運動の中枢制御およびその病的状態を随伴陰性変動(CNV)あるいは運動準備電位(BP)という脳電位の点から研究することを最終目標として、まずCNVそのものの発生機序を検討し、さらに歩行を含めた随意運動が傷害される運動異常症においてCNVあるいはBPを検討してその病態を検討した。
1.パーキンソン兆候を有する患者(パーキンソン病・進行性核上性麻痺)13名においてCNV後期成分とBPを比較検討した。Hohen & Yahr分類でII度以下の軽症群ではCNVとBPに有意な差はなかったが、III度以上の重症群ではCNVはBPより有意に低振幅化していた。さらにBPが正常に記録されるパーキンソン病患者(Hohen & Yahr分類III度)において、CNV後期成分が頭蓋頂(Cz)においてのみ低振幅化し外側半球領域では正常に認められる所見が3名でみられた。これらの事実より、(1)CNV後期成分とBPは発生機序が異なり、CNVの方がより基底核傷害の程度を反映する、(2)CNV後期成分の発生源の1つが補足運動野(SMA)であるという著者の過去の報告はパーキンソン病患者でも矛盾せず認められ、特にCNV後期成分の低下はSMAから始まることが示唆された。
2.CNVの1対の刺激に対して、S1で運動を選択しS2で運動を開始する場合(S1選択反応課題)と、S1は警告のみでS2で運動を選択・開始する場合(S2選択反応課題)で、S1から約500msで出現するCNV前期成分の分布を比較して発生機序を検討した。S2選択反応課題でのCNV前期成分は前頭部正中領域に陰性電位として見られたが、S1選択反応課題でのCNV前期成分は、より前方の陰性電位と頭頂部やや左の陽性電位の複合として認められ、これは前頭前野および頭頂連合野での判断・選択の過程を反映する可能性が示唆された。
3.心因性ミオクローヌス(psychogenic myoclonus)と診断された患者で観察される"不随意"運動では、運動に先行するBPが記録され、皮質レベルでは随意運動と同じ準備状態を呈する。本検査は所見が陽性である場合に限り、本病態の診断上臨床的に有用と考えられる。
