研究概要	ヒトの随意運動に関する高次脳機能の病態生理学的・無侵襲的研究の初年度(平成7年度)は主に次の様な研究を行い、それぞれ新しい知見を得た。 (1)運動関連脳磁場と皮質背景活動の変化との関連正常者が一側の手指を随意的に運動させる約1秒前からは、脳電位として陰性緩電位が出現するが、脳磁場上も同様の緩徐な変化が運動と対側優位で両側の一次運動感覚野から出現した。さらにtemporal spectral evolution(TSE)という解析方法によって同時に検討した皮質背景活動では、10〜20Hz帯域の速波成分の減少が緩徐成分変化よりもむしろ先行して出現し、しかもそれは明らかに運動とほぼ対側の一次運動感覚野に限局することが明らかにされた。これらは、随意運動の発現機構は一次運動感覚野内でも緩徐成分と速波成分とは異なる神経機構を反映することを示唆する。 (2)感覚運動連関に及ぼす基底核の影響まずてんかん患者の脳内電極からの直接記録で、外部刺激に反応して随意運動を施行する際の脳電位(随伴陰性変動:CNV)の発生源を明らかにした。基底核の機能異常であるパーキンソン病患者のCNVの振幅は症状の程度に相関した。また基底核障害では歩行異常は重要な症状であるため正常者において歩行運動に先行するCNVを検討し、単純な足の運動と比較した。歩行時には正中部で選択的にCNVが増大し、補足運動野の活動との相関が強く示唆された。また随意的な筋収縮の弛緩に先行する準備電位を正常者で検討し、筋収縮の場合と同様の脳内準備状態が生じることが示唆された。同様の課題条件でポジトロン断層撮影(PET)による脳血流変化を測定し、一次・補足運動野以外に、内側前頭前野の関与も示唆された。また右上肢ジストニア患者での検討では、筋収縮の弛緩に先行する電位は左半球でより低下している傾向がみられた。ジストニア患者でもPETによる脳血流変化を検討中である。
