太陽紫外線誘発皮膚癌の臨床疫学的研究-本邦人における危険因子とその対策-

研究概要	健康人を対象とした皮膚癌検診を兵庫県加西市(人工約5万人)、宮崎県清武町(同2万5千人)および沖縄伊江村(同2400人)で実施した。それぞれの地域の検診結果から人工10万当たりの日光角化症(SK)有病率を求めると、加西市203.9、清武町644.6、伊江村では70歳代の弾性有病率で14,545と著しく高い値を得た。加西市検診の結果から、日光曝露部に良性腫瘍が6個以上多発することSKの危険因子であることが示唆された。化粧とSK有病率の解析から、化粧は皮膚癌予防に有用であることが示唆された。全国5ケ所の太陽UVB計測では、1993年から地表紫外線B(UVB)は増加していないことが示された。太陽光線で生じる(6-4)光生成物は速やかにDewar異性体へ移行することがアガロース法で確認された。色素性乾皮症患者に生じた有棘細胞癌(SCC)の腫瘍の大きさ及び悪性度(未分化度)とp53遺伝子突然変異の関係から、p53異変が腫瘍進展および腫瘍増大に関与することが示唆された。SCC患者リンパ球DNAのN^7-メチルグアニン量は6.6【minus-plus】5.3/10^7グアニンと健康ヒトの23.2【minus-plus】13.8/10^7グアニンに比べ有意に低いレベルを示した。皮膚癌患者ではUVB照射により95%の高率で皮膚の免疫制御が誘導され、健康人の5%に比べ明らかな差異を示し、紫外線による免疫制御が発癌に関与する可能性が示された。T4N5リポソーム塗布はマウスの紫外線発癌を有意に遅らせた。T4N5の臨床応用が期待される。将来UVBが増加すれば高齢化社会の日本で皮膚癌が増加することが強く示唆された。今後、遮光を含めた皮膚癌予防対策が重要と考えられる。
