研究概要	1.遺伝性非腺腫症性大腸癌(HNPCC)の原因遺伝子の検索日本人HNPCC患者についてDNAミスマッチ修復遺伝子hMSH2とhMLH1の変異を検索した。その結果、2つの遺伝子の変異が複数検出され、日本人でもhMSH2とhMLH1が主であることがわかった。また、HNPCCでも遺伝子診断が可能となった。 2.HNPCCの癌化機構 HNPCCの癌だけではなく腺腫でも、RER(replication error)とTGF-βII型レセプター遺伝子中のAのくり返しに異常が高率に検出された。故に、HNPCCでは腺腫から癌化することが多いことが示唆された。このことから、HNPCC患者及び変異遺伝子保有者は、定期的に大腸・直腸の検査を受けて腺腫が見つかり次第除去することにより癌発症を減少できる可能性が期待できる。 3.遺伝性大腸癌の遺伝相談家族性大腸腺腫症(FAP)とHNPCCの遺伝子診断を臨床に導入するため、医師及び看護婦が患者や家族と面談して、癌の予防学的行動を患者あるいは家族がとりやすくする要素、あるいはそれを阻害する要素はなにかを分析した。その結果、情報提供、悩みの受け入れ態勢があることなどが重要であり、カウンセリングのできる看護部門の確立が必須であることが明らかになった。
