研究概要

歯周疾患を考える上で必須である歯周ポケット内細菌叢の生態を理解し、これをコントロールするために、以下のような検討を進めた。
1。歯周ポケット内細菌叢構成細菌の特徴の解析。
(1).偏性嫌気性菌が多いこと。(2).糖非分解性の細菌が多いこと、(3).この様な性質を持つグラム陽性桿菌の新しい菌種が数多く発見されたこと。(4).中性条件を至適とする細菌腫が多く、酸抵抗性が低いこと。(5).ポケット内グラム陰性桿菌のイオン要求性として、ナトリウム塩が考えられること。歯垢の酸産生菌では、むしろカリウム塩要求性である。
2。ポケット内細菌叢細菌の病原性の解析。
(1).細菌のポケット内定着に関連して共凝集能、酵素活性、線毛等があること。(2).ポケット内細菌の免疫反応への関与(抗体産生能、免疫細胞活性化能等)とその反応による組織破壊等も別途検討された。
以上の検討を受け、生態学的コントロールとしては、以下のコントロールが可能と思われた。
1。ポケット内微小環境の変更による細菌数と細菌の質のコントロール。
(1).好気環境への変更;特定の細菌の増殖、酵素活性、毒素産生の抑制が見られた。(2).酸性環境への変更;多くの偏性嫌気性、糖非分解性の細菌の生育を抑制した。(3).環境変更による種々の酵素活性の変化が示され、生態学的コントロールの可能性が示された。
2。抗菌物質によるポケット内細菌数のコントロール。
植物性の抗菌物質、抗生物質等の適切な使用によるポケット内細菌のコントロールが可能と思われた。
3。ポケット内細菌の病原性要因のコントロール。
前述のような種々の病原性要因の産生、発現機構が検討された。
4。ポケット自体の消減。
将来的な検討として、歯周ポケットそのものの消滅を図る検討も重要になると思われた。
