研究概要	坦癌肝区域のNon-touch isolationによる残肝の再移植手技に関する実験では手縫い吻合では困難であった肝動脈吻合に血管吻合クリップを使用する方法をとるようにして開存率が上昇し、また、術中、術後の凝固系異常活性化への対策として、手術中よりDIC機転のtriggerを引かないようProtease inhibitorを持続点滴をおこなった。その結果、再移植後の出血制御が極めて良好となり、動物は10日以上生存するようになった。残る問題点は複雑な手術手技と人工血管を使用するため、長期成績改善のためには感染の問題が残っている。これは人では比較的容易と思われるが動物実験上は難題である。障害肝に対する熱ショック前処置の効果に関しては平成7年度にCCl_4による急性肝不全モデルにおけるその保護効果を証明したが、平成8年度には潅流モデルでCCl_4と同様にフリーラジカル障害をおこすButylhydroperoxideを摘出肝に潅流し、熱ショック群と非熱ショック群で比較した。この結果、熱ショック蛋白の存在はフリーラジカルによる脂質過酸化や、さらなるラジカルの発生を抑制するよりも、むしろ生じた障害の回復過程に意味を持つらしいことが判明した。追加研究が必要であるが、本研究の目的とした対外抗癌療法における抗癌剤潅流時における熱ショック蛋白の存在意義に関する重要な所見となった。さらに、アドリアマイシンの投与そのものが一つの前処置としてHSP72を肝に誘導する効果があることが抗癌療法による肝障害実験の結果として判明し、現在この新しい前処置により得られたストレス反応が虚血再潅流障害に対する保護効果を付与するために役立つかどうかを検討中である。将来の術前処置として熱ショックに替わるさらに低侵襲的で実際的な方法開発への発展が見込まれる。Non-touch isolation手術への実際の熱ショック蛋白の応用は今後の課題であるが、今回のプロジェクトでその基礎データが集積された。
