精神分裂病と感情障害の発症と経過に関与する神経伝達物質関連遺伝子変異の検索

研究概要	ドーパミンD2受容体遺伝子の変異S311Cについて精神分裂病と感情障害の患者における頻度と症候学的特徴をまとめて発表した。分裂病患者291名、感情障害78名、健常対照者579名について検索し、次の結果を得た。分裂病では26例(8.9%)がこの変異を有し、対照における頻度(4.1%)より有意に(p<0.01)高かった。特に陰性症状のない分裂病者では17.1%の患者がこの変異を有していた(p<0.00001)。これに対し、陰性症状を持つ患者群では頻度は5.7%であり、対照群と差がなかった。30歳以上の分裂病患者で婚姻歴のある者について、S311Cの有無で比較したところ、特異を持つ患者の婚姻歴(58.8%)は特にない者のそれ(27.3%)より有意に(p<0.02)高かった。まだ症例数は少ないものの、感情障害でも興味深い結果が得られた。感情障害78例中この変異は7.7%にみられたが症候学的にみて気分と一致しない精神病症状を有する患者に33.3%という高頻度にこの変異が認められた(p<0.01)。アルコール依存症についてもこの変異を持つものが対照群より高いことを確認した。今年度はアルコール依存症317名について、チロシン水酸化酵素5'領域T-229A変異、イントロン1VNTR、エキソン3およびエキソン14のMet81変異について関連研究を行ったが、明瞭な頻度差はみられなかった。なお、未服薬および服薬中断分裂病17例についてPETを用い^<11>C-SCH23390でD1受容体を測定したところ、前頭前野で結合活性が低下しており、陰性症状の強いものほど低下が顕著であるという結果を得たので、分裂病死後脳で現在^<14>H-SCH23390を標識化合物としてD1受容体を測定している。
