全身性炎症反応症候群(SIRS)としての急性膵炎とその重症化機序の解明

研究概要

急性膵炎臨床例における全身性炎症反応症候群(systemic inflammatory response syndrome : SIRS)の発症率は、軽症で22%、中等症で67%、重症で92%であった。またSIRSの7日以上の持続、及びSIRS陽性項目数3以上で、そけぞれ致死率35%-39%と有意に致死率が増加しており、SIRSの程度は急性膵炎の重症度を反映していた。特にSIRS陽性項目数3以上という因子は発症早期に把握が可能であり、重症度判定に有用であった。SIRSは高サイトカイン血症の臨床的反映であることから、このことはサイトカイン反応が膵炎の重症度に影響することを意味する。
また、ラットセルレイン膵炎を用いて重症化機序の解析を行った。膵外分泌刺激物質であるセルレイン(Crn)を投与した単純膵炎群(Crn群)、膵炎を起こした後、さらにlipopolysaccharide(LPS)を投与した感染膵炎群(Crn+LPS群)、及びLPSのみ投与したLPS群で比較検討し、以下の結果を得た。(1)膵炎の発生に伴い肺胞マクロファージがプライミングされた。この状態にLPSを投与し感染状態を誘導すると、マクロファージからの好中球に対するケモカインであるCINC(cytokine-induced neutrophil chemoattractant)が産生され、それに伴って好中球が肺内に集積、浸潤した。(2)肺障害の解析として、肺のコンプライアンスを測定した。Crn+LPS群においては、Crn群、LPS群に比し有意に肺のコンプライアンスが低下していた。すなわち、好中球の肺への集積により肺障害が発生したことが確認された。(3)以上の反応は、炎症性サイトカインの産生を抑制するメチルプレドニゾロンやプロペントフィリン、あるいは好中球エラスターゼ阻害剤の投与により軽減した。
以上により、膵炎の重症化にはサイトカイン反応にひきつづく全身の炎症反応が関与すること、特に感染により反応が増強され臓器障害が生じやすいことが明らかとなった。
