新しい心不全治療薬開発の試み

研究概要	慢性うっ血性心不全の患者は現在世界で約1000万人おり65才以上の入院を必要とする患者の疾患としてはもっとも多い。死亡率は年20〜50%である。しかし現在のところ確立された治療法はない。心不全治療薬は(1)心筋に対する負荷の減少と(2)心筋細胞収縮性の増強である。(2)の機序で作用する強心薬は血行動態と運動能力の改善により患者の生活の質(QOL)を顕著に改善する。しかし臨床試験において長期的投与により生命予後をプラセボよりもむしろ短縮するという結果となり、現在はもっぱら(1)による治療が主体となっている。従来の強心薬はスベテ細胞内Ca上昇により強心作用を発揮するので、これらの強心薬の組み合わせはCa過剰負荷による心不整脈の発生、エネルギー効率の低下、および心筋細胞障害を惹起しやすい。さらに心筋虚血再潅流後に生ずる気絶心筋(stunned myocardium)のような状態では、細胞内Ca上昇はもはや心筋収縮力を改善することが不能である。そこで現在ジギタリスとカテコールアミンに代わりうる強心薬として収縮タンパクCa感受性増強機序により強心作用を発揮するCaセンシタイザーが心不全治療薬として非常に期待されている。本研究では、Org30029がアシドーシスあるいはBDM (E-C uncoupler)の存在したで起こる収縮抑制を効果的に改善することをエクオリン負荷イヌ心室筋標本で明確に示した。本化合物は残念ながら効力(potency)の低さから臨床的な投与は見合わせられている。さらに新しいCaセンシタイザーとして、(1) levosimendan, (2) MCI-154, (3) SCH00013など強心薬の細胞レベルにおける作用機肋が解明された。(1)と(2)に関しては現在その臨床試験において効果が検討されている。(3)については、基礎的な研究が終了した段階であるが、心拍上昇を起こさないCaセンシタイザーとしてその臨床効果が期待される。
