生体肝移植後免疫抑制療法離脱の指標となる評価法の確立と離脱の試みに関する研究

研究概要

肝移植後の免疫抑制療法の戦略は、効果的な免疫抑制と共に副作用防止にあり、目指すところは、免疫寛容を獲得して免疫抑制剤の離脱にある。そこで計画的に免疫抑制療法の減量を試み、その指標となる研究を行った。
1)3期にわたり、タクロリムスの減量プロトコールを実施した。血中濃度に基づく、経口投与が至適療法と判明した。
2)ステロイドは1年で90%が離脱できた。ステロイド離脱の早さは、タクロリムスの効果と生体肝移植の利点によるものと考えられた。
3)200例の生体肝移植後、約85%の生存率を得た。このうち7例に免疫抑制剤を完全に中止し得た。4例は、EBウイルス感染後の症例でEBウイルスが免疫寛容に関与した可能性がある。
4)6カ月、1年と長期経過症例の肝機能検査、および肝組織像では、約10%に軽度の異常所見を得た。この点の解明と治療が今後の問題である。
5)生体肝移植患者におけるリンパ球混合培養上清中サイトカインの検討
肝機能が安定している肝移植後患者22人の末梢血リンパ球混合培養上清中サイトカインを測定し、対ドナー・対third-party間の反応の差、Th1・Th2系サイトカインの変動を、タクロリムスの用量によって3群:0.5mg/kg/週以上(6人)・未満(13人)・(3人)で比較検討した。ドナーに対するTh1系サイトカイン(IL-2,IFN)放出量は、hird-partyに比較し統計的に有意に少なく、しかも、タクロリムスの量が減少するにつれて増大した。Th2系サイトカイン(IL-8,10)放出量には一定の傾向は認められなかった。従って、免疫抑制剤の減量または中止に成功し得た症例において、末梢リンパ球のドナーに対する反応が増強しており、免疫寛容に、移植臓器内の局所的なリンパ球抗原認識能または細胞障害機能の低下が関与することが示唆された。
